This page explains what information this website collects and how it’s used. It applies to this site only, not to care provided at Bloom Aesthetic Surgery Group or The Surgical Arts Center.
When you submit the consultation request form, we collect your name, contact information, the procedure(s) you’re interested in, how you heard about the practice, and anything you choose to share in the message field. We don’t collect health information through this website — that’s discussed directly with you at your consult.
Information submitted through the consultation form is used solely to follow up with you and help get you scheduled. It is not sold, rented, or shared with third parties for marketing purposes.
Form submissions are processed through Formspree, a third-party form service, and are accessible to our office for the purpose of scheduling. Please avoid including sensitive medical details in the form — we’ll cover all of that directly with you.
